Our Story

HOME & COMMUNITY CARE

Home and community care services support individuals needing assistance with daily activities due to aging, illness, or disability. These services enable clients to maintain independence in their own homes or communities, offering help with personal care, medication management, CADI Waiver and companionship.

ELIGIBILITY:

Must be 18 years or Older.

Housing Instability need for Services.

Must be on Medical Assistant (MA)
